I give permission for my minor child to be photographed and/or recorded by NASA or its representatives. I understand that the resulting images, audio, and/or video may be used in the production of instructional or promotional materials created by or on behalf of NASA. These materials may be publicly shared in various formats, including online, broadcast, or printed media.
This permission is granted for an unlimited duration, and I acknowledge that neither I nor my child will receive any compensation for the use of these materials. I understand NASA is under no obligation to use my child’s likeness or voice and may edit the content as necessary. I also waive the right to inspect or approve the use of the content now or in the future.
I release NASA and its representatives from any and all claims arising from the use of my child’s image, voice, or name under the terms of this agreement.
By submitting this form, I affirm that I am the legal parent or guardian of the minor listed below, am at least 18 years of age, and have the legal authority to grant this permission.
